Re-issue of Passport for applicants below 18 years of age:

  • Four photographs (5cmX5cm)

  • Current passport in original and copies (first & last page)

  • Copy of Residence Permit/Work permit (front and back side)

  • Photocopies of both the parents'- 

    1. Passport (first and last page) 
    2. CPR card 
    3. Residence permit

  • Copy of CPR card

  • A self -addressed stamped DKK envelope of appropriate size if you want the passport to be posted back to you

  • Proof of payment

  • A declaration affirming the particulars furnished in the application about the minor as per Annexure D.

  • A Declaration affirming the particulars furnished in the application about the minor as per Annexure C (one parent not given consent /Annexure D cannot be arranged )
